NetzwerkPflege_C: Networking

NetzwerkPflege_C(hemnitz) was founded on 1 July 2009 as part of the "Networked care advice" pilot project as a Saxon alternative to the care support centres. The aim was to further develop the nursing care structure in such a way that a far-reaching and structured linking of all providers and services active in the field of nursing, care, support and counselling could be achieved in order to provide local and customised help for people in need of support.

To this end, stakeholders from the care insurance funds, the social welfare office, other authorities, public, independent and private organisations and services have joined the NetzwerkPflege_C.

The Care Coordination Office in the Local Care Authority, Social Services for Adults department of the Social Welfare Office is responsible for organising the Care_C network. The coordination centre is the point of contact for all matters concerning the network partners, the admission of new network partners, the initiation of network conferences and the provision of information to members.

Become a member

The NetzwerkPflege_C(hemnitz) can be joined by natural persons, legal entities, self-help groups, public, private, non-profit or other organisations who declare their membership by means of a legally binding signature on a declaration of membership (link) and recognise the guidelines of the NetzwerkPflege_C (link). Membership is not tied to a membership fee. However, it is expected that every member represents the interests of people in need of care and actively participates in the network's work.

The admission of new members is discussed in the founding working group and decided in a democratic voting process.

Networking

Structurally, the NetzwerkPflege_C is divided into the preparatory committee for the organisation of network conferences and into topic-related working groups that act independently.

Volunteering working group

Volunteering is an important resource in an ageing society and deserves to be recognised, appreciated and protected. To this end, it is necessary to define basic framework conditions. The "Volunteering" working group developed a guideline for volunteers and civic engagement in Chemnitz. The working group's activities have been completed.


Care counselling working group

In the "Care counselling" working group, counselling standards were developed for cooperation between the network partners in order to advise, guide and refer those seeking advice according to their specific concerns. The working group's activities have been completed.


Interfaces working group

In the "Hospital and care facility interfaces" working group, transfer standards and transfer documentation were developed for the structured transfer of patients or residents to the hospital or back to the care home. The working group's activities have been completed.

The working group also dealt with the project "Medical care in nursing homes". The starting point here was that nursing home residents no longer had a GP due to the shortage of doctors, meaning that the emergency services had to become active. The aim was to test a model of medical care in nursing homes. Unfortunately, this could not be realised for various reasons.


Working group on care services

The "Care services" working group focussed on collecting data on care services in Chemnitz. The initial aim was to create a separate database for the NetzwerkPflege_C. After the Free State of Saxony provided the PflegeN as a basis, the collected data was incorporated into it and can still be accessed by all citizens today. The work of the working group has been completed. Data maintenance is now the task of the care coordination department.

AG Recruitment of skilled labour and young talent

The working group endeavours to address the issue of the shortage of skilled workers and young talent in the care sector. Every year, an action day is organised in the form of a white professions fair. The main target groups are pupils in the career orientation phase and adults who want to reorient themselves professionally. Experts from the field and representatives from the employment agency present themselves at exhibition stands and provide comprehensive advice on training, qualification and career opportunities. Admission is free and registration is not required.

A network conference is generally held annually with the partners of thePflege_C network. This provides information on new care-related specialist topics, developments and legislation. The active working groups also present the results of their work. Network activities are analysed.

New members who have applied to join NetzwerkPflege_C present themselves and their services. The network conference then votes on their admission.


Network conference 2024

On Wednesday, 24 April 2024, the 17th Network Conference of NetzwerkPflege_C took place in the ballroom of Chemnitz Hospital on the Dresdner Straße site. Around 90 participants discussed a wide range of topics.

Looking back on the 2023 network conference, Ms Claudia Schöne from AOK PLUS spoke about the current status of staffing levels in nursing care.

The topic of hospice and palliative work at the interface with care in Chemnitz, which was communicated by the representatives of the hospice and palliative services, was new.

Architect Susanne Trabant discussed whether smart aids can make everyday care easier as innovative solutions.

The generalist nursing training programme is a perennial topic at the network conferences. Mr Martin Fritzenwanker from the Saxony Nursing Training Advice Centre discussed the reasons for training drop-outs and the challenges of practical guidance. The input formed the basis for the subsequent panel discussion with representatives from theory and practice.

Network conference 2023

On Wednesday, 3 May 2023, the 16th Network Conference of NetzwerkPflege_C took place in the ballroom of Chemnitz Hospital on Dresdner Straße. Around 70 participants discussed the topics of new staffing levels in inpatient care facilities and generalist nursing training. Sigrun Pohl, a consultant at the Saxon State Ministry for Social Affairs and Social Cohesion, spoke with Claudia Schöne, Head of Division at AOK PLUS, about the opportunities and challenges of staffing levels in inpatient care facilities from 1 July 2023.

Representatives from the medical vocational schools, Chemnitz Hospital, medicampus Chemnitz and BIP Chemnitz gave a review of how the first graduates coped with the generalist nursing training programme.

Lucie Strauß and Claudia Küttner from the Chair of Health and Nursing/Vocational Didactics at TU Dresden presented the model project "Saxony Nursing Training Advice Centre".

In the afternoon, the participants were able to become active themselves in three workshops on the above-mentioned topics.

After the presentation of the workshop results, it was possible to look back on an interesting and constructive network conference.

Network conference 2022

On 18 May 2022, after a long break due to the coronavirus, the 15th Network Conference of NetzwerkPflege_C took place in the ballroom of Chemnitz Hospital at the Dresdner Straße site.
At the beginning, Ms Claudia Schöne (AOK Plus) gave an insight into the innovations in care and current challenges. Ms Anett Peuthert (BAfzA) then introduced the participants to the topic of "Two years of training under the Nursing Professions Act - challenges, milestones, outlook". After the presentation of the ZenKoPa coordination platform by Mr Markus Müller (ZenKoPa GmbH), all participants were able to fortify themselves with a shared lunch. The event was rounded off with an interesting presentation by Ms Juliane Mentz (AGAPLESION MITTELDEUTSCHLAND gGmbH) on the topic of "Digital recruiting - how Facebook, Instagram, YouTube and Google support employee recruitment" and stimulating discussions at various themed tables.

In cooperation between the Deutscher Verband der Leitungskräfte von Alten- und Behinderteneinrichtungen e. V. (DVLAB), the Saxony state working group of the Bundesarbeitsgemeinschaft leitender Pflegepersonen e.V. (BALK e.V.) and the social services department of the city of Chemnitz, the first care managers' get-together was launched in Chemnitz on 9 September 2010. Since then, the regulars' table has taken place every second Wednesday in January, March, May, July, September and November from 4.00 to 6.00 pm.

The care coordinators invite you to the regulars' table and inform you about the thematic input. Afterwards, all interested managers and professionals can take part in an open exchange of experiences.

Based on the principle of "best practice", there is an active exchange between participants to discuss current topics and find possible solutions to problems. In this way, everyone can benefit from each other and continue to work together on uniform standards that contribute to the well-being of residents, clients and patients.
